    • In some locations, you may be able to make an appointment online so you don’t have to wait in line.
  2. Obtain an application from the attendant. Fill out all of the required information, including your name, birth date, and the last 4 digits of your Social Security number. You may also need to provide your zip code.[14]
    • Alternatively, visit your local DMV’s website to download the form. Fill out the form before you go to the DMV.
