If yes, they might have accepted your request by accident, and therefore changed their mind and removed you. If a private user removes a user from their follower's list, the removed follower will no longer be able to view the private user's content. If some images on their profile simply fail to show up, it's either a glitch within the site or your device is very slow or overloaded at the time you're viewing the profile. It's definitely not blocking or restricting; blocking a user means they cannot see the page at all and restricting means they can still see everything on the page (if it's public) but can't comment on the posts.
