    Buy a belt that's one size bigger than your waist. So, if you have a 32 waist, you'd get a 34 belt; if you have a 34 waist, you'd get a 36 belt.

Clockwise means that you start at the loop just to the right of the button and end at the loop just to the left, whereas counterclockwise is the other way around.
